CALL FOR APPLICATIONS
The Delta Dental Foundation is pleased to announce a Call for Applications for its 2010 Community Mini-Grant Program. A total of $100,000 will be awarded to assist in addressing the oral health needs of children in Michigan, Ohio, and Indiana.
Community-based oral health programs in the tri-state area are encouraged to apply for grants up to $5,000 for programs designed to improve oral health. Special consideration will be given to those applicants whose programs focus on improving the oral health of low-income children, however, all grant submissions will be considered. To be considered for the 2010 program, the Delta Dental Foundation must receive completed contribution request applications no later than July 16, 2010. Grant recipients will be notified later this year.
To apply for a mini-grant, please complete the Delta Dental Foundation Community Mini-Grant Program Contribution Request Application and include any information to support your request. Please electronically transmit your application and supporting information by attaching it to an e-mail. If preferred, you may mail or fax your completed request to the address below. Only one application per organization will be accepted.
