Delta Dental has the largest network of dentists across the U.S. and Puerto Rico. Search for a dentist using our online dentist directory 24/7. You can also find a dentist by calling customer service at (800) 524-0149.

 

What’s the Consumer Toolkit?
Consumer Toolkit gives you easy, secure access to your benefit information 24 hours a day, seven days a week. Review eligibility, annual maximum used to date, levels of coverage, and claims information. You can also search for a dentist, print your your member ID card and sign up for electronic EOB statements using this innovative tool.

To access the Toolkit, you will need to register first. To register you will need the following:

  • Subscriber's first and last name
  • Subscriber's date of birth
  • Member ID (use Social Security number)
  • Security question

Print your own member ID card.
To print an ID card, you will need to log into Consumer Toolkit. Once logged in, click the Print ID Card link on the lefthand menu. You will see your member ID card displayed on the screen. Use your browser's print function to print as many copies as desired.
  Need a claim form?
Remember, when you visit a participating Delta Dental dentist, the dentist will complete and file claim forms for you. If you choose to visit a dentist who does not participate with Delta Dental, you may need to file your own claim forms.
